Call Required: Yes
Weekend Required: Yes
Call is Off-Site Beeper Call with hourly call back if provider is called back in. Weekdays are 3p-7a and weekends/holidays are 24 hours. 

Site Details:
Length of need: July and August FT coverage
Average Time to Credential: 30 days Emergency Privileges available Clean Files ONLY  
Types of Cases: General surgery, OB/GYN, GI, Ortho, and occasionally CathLlab. No hearts or neuro.
Majority Case Type: Ortho and General
# of Operating Rooms: 2 main operating rooms
Shift Days: Weekdays Monday thru Friday 
Shift Times: 7a-3p 
Minimum Number of shifts per month: 10-20
Call Required: Yes
Weekend Required: Yes

Qualifications:
Must have active AZ Medical License
Must have clean malpractice background no background issues
Must have Active AZ License
Board Certified Required
Must submit CRNA Skills Checklist with Presentation
Clean malpractice 
Availability of at least 3 wks per month

Contract Term:
